Elon keeps failing. After firing most of the (already underpaid and overworked) FAA staff, and anybody he can from any agency that watches what his businesses do, his failure explodes again.
Yes people fail, yes failure happens. But that is why NASA and ESA build into their systems redundancies- not just in hardware, but in checks and verification / certification. Sometime it’s better to have many people to check something, and multiple agencies check something, than to have 1 or a few. Do accidents still happen? Sure.
